Homeless Housing Assistance and Prevention (HHAP)

 
 

Program Description and Eligibility

HHAP serves youth who are currently homeless. A variety of services are provided through case management in order to assist youth in meeting their basic needs and/or achieve reunification, shelter or housing. Each youth creates an Individualized Service Plan with their Case Manager along with a Housing Plan. The Housing Plan consist of an emergency housing, transitional housing and/or permanent housing goal.

Youth (18-24 years old) who are homeless in North County.


Program Services

  • Prevention and Diversion Services

  • May consist of diverting and reunifying youth with family if safe and appropriate

  • Providing crisis response services to address immediate non- housing related needs, including connecting youth to local resource

  • Assisting in acquiring basic needs such as food, drink, clothing, hygiene products, transportation, health insurance, child care, and identification and documents. Also includes providing housing assistance.

  • Client Flexible Financial Assistance. *As funds are available for clients that meet additional eligibility criteria.
